How we use your information
Under the Data Protection Act 2018 Wolds Wellbeing are legally obliged to update you as a customer to inform you how I hold and process your personal information and details.
As a sole trader I am responsible for the control and processing of this information.
I do not and will not share information / data with third parties unless i have the permission of the individual to release it.
Health questionnaire - I will have asked you to complete a form prior to attending a class or massage treatment. This form holds your personal information including health history past and present.
Health questionnaires are very important and essential for my business as they provide key information for structuring my classes and massage treatments to meet the needs of the individual / client.
I am bound within my Public Liability Insurance to obtain each client's details in the event of a claim. By law I have to store your details for 7 years and are available for you to view should you require it.
Other then for reference and contact details - nothing
I will delete and dispose of customer details with the appropriate methods. I will shred all information on that individual by means of a cross shredder and dispose of the paper in relevant paper disposal units or burn it.
You are required to provide, complete and sign to the terms and agreement stated within the business above and sign the disclaimer.
